Adult Mental Health Case Manager
Maine Behavioral Health Organization (MBHO) is seeking a full-time Adult Mental Health Case Manager to join our dedicated team serving the Augusta, Farmington, and Skowhegan areas. This role is ideal for compassionate professionals who are committed to empowering individuals on their recovery journey by providing direct mental health/co-occurring services, advocacy, and access to resources in a community setting.
Position Overview
The Adult Mental Health Case Manager works as a trained and supervised professional within a team of behavioral health experts, providing personalized services that support individuals in symptom management, accessing community resources, and achieving greater independence. This position involves work in clients’ homes, schools, and community settings, offering a high level of autonomy while collaborating with team members and external stakeholders.
Essential Responsibilities
- Provide direct support, advocacy, and oversight to individuals served in alignment with their treatment plans.
- Assess, evaluate, and develop individualized plans of care tailored to the needs of the individual.
- Establish and maintain community links, including referrals, coordination, and advocacy, to facilitate access to needed services and supports.
- Build and maintain strong working relationships with stakeholders, including guardians, provider agencies, and community vendors.
- Ensure respect for the rights, privacy, and dignity of individuals served in all interactions and activities.
- Meet or exceed all documentation and service standards, including Individualized Service Plans (ISP), assessments, census reports, and continuing stay reviews.
- Ensure compliance with regulatory, contractual, and agency obligations.
Qualifications
- Education: High school diploma or GED required; an associate degree in a related field is preferred.
- Experience: A minimum of 6 months of related experience is desired.
- Certification: Certification as a Mental Health Rehabilitation Technician - Community (MHRT/C) or a Provisional MHRT/C is required. Ongoing maintenance of certification with 18 approved CEUs every two years is mandatory.
- Licensure: Valid state-issued driver’s license and proof of current insurance are required. Reliable transportation is also essential.
- Skills: Strong organizational, communication, and interpersonal skills, with the ability to work both independently and collaboratively.
