We are looking for a Care Coordinator with strong skills to join our Portland BHH team. You will work with the children of families within our catchment area based out of the Portland office which covers Cumberland County. You will join a team of Care Coordinators, family support persons, clinical leads, and medical/psychiatric/and nursing supports. We are looking for someone with the ability to manage a busy schedule, the needs of each family, and documentation needs.

DUTIES:
The Care Coordinator will complete psychosocial assessments and develop plans of care for youth with the support of caregivers. The Care Coordinator oversees the implementation of the plan of care and documents progress on goals. In this position, you will support refugee youth with accessing community resources, access to medical care, education, recreational activities, and basic needs.
QUALIFICATIONS:
Bachelor's degree from a 4-year institution of higher learning. Experience doing cross-cultural work and/or experience with traumatized children and families is preferred. All applicants must maintain a current driver's license and a clean driving record as outlined in Spurwink's Driving policy.
