Job Description
Will be responsible for managing and coordinating comprehensive care for clients within the CCBHC framework. This role serves as one of the first contacts for individuals seeking services at the CCBHC that are unhoused. This role involves conducting screening, initial assessments, providing crisis intervention, and coordinating immediate care needs to ensure clients receive timely and appropriate services. After intake, this person will act in the role of a case manager, aid in service coordination, referral, and supportive counseling. This position is primarily office based, with some expectation of community engagement depending on the workflow of the day. This role works collaboratively with interdisciplinary care teams to ensure clients receive the necessary services and support to improve their health outcomes and quality of life.
