Overview
We are seeking a dedicated and knowledgeable Certified Diabetes Educator to join our healthcare team. This role involves providing comprehensive diabetes management education, supporting patients in achieving optimal health outcomes, and collaborating with multidisciplinary teams. The ideal candidate will possess strong expertise in physiology, nutrition, and care planning, with experience in various healthcare settings including hospitals, long-term care, and dietary departments. This is a vital position aimed at improving patient quality of life through personalized education and support.
Duties
- Conduct patient assessments to evaluate individual needs and develop tailored care plans for diabetes management.
- Provide expert health coaching on blood sugar control, medication adherence, lifestyle modifications, weight loss strategies, and nutrition.
- Educate patients on diabetes-related topics such as physiology, diagnostic evaluation, tube feeding, and long-term care management.
- Supervise and support care teams in implementing effective diabetes education programs.
- Utilize EMR systems to document patient interactions, progress notes, and care plans accurately.
- Assist with dietary planning and collaborate with dietetics professionals to optimize nutritional support for diverse patient populations including geriatrics and acute care settings.
- Monitor patient progress through ongoing assessment and modify care strategies accordingly.
- Educate patients about the use of insulin pumps, glucose monitoring devices, and other medical equipment related to diabetes management.
- Support patients in weight loss initiatives and chronic disease management programs.
- Stay current with advances in diabetes care, nutrition science, and healthcare regulations to ensure evidence-based practice.
Experience
- Proven experience in health coaching within clinical or community settings.
- Supervising experience preferred, with a background in managing healthcare teams or educational programs.
- Extensive knowledge of physiology, nutrition, dietetics, and diagnostic evaluation related to diabetes care.
- Prior experience working in hospital environments or long-term care facilities is highly desirable.
- Familiarity with EMR systems for documentation and patient record management.
- Experience with tube feeding protocols and developing individualized care plans for complex cases.
- Background working within dietary departments or nutritional support services is advantageous.
- Knowledge of weight loss strategies and patient assessment techniques for diverse populations including geriatrics is beneficial.
