Case Managers/Care Coordinators works with children and their guardians to assist them in identifying their strengths and needs; facilitates the Child and Family Team to develop a Plan of Care to address the child's and family's needs. We make referrals, monitor and coordinate services and work with providers and natural supports. This position is based in the office/remote work setting, the client's home and in the community. A bachelor's degree is required, as well as a drivers' license and reliable transportation.
