SUMMARY
The Rehabilition Specialist coordines activities for residents of care and trement facilities. Assesses client needs and designs and implements rehabilition programs.
ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to sisfactorily perform each essential function listed below.
- Performs skill training, cognitive remediion, job coaching, and behavior management objectives in accordance with trement plan.
- Reinforces approprie client behavior and skills in residential, community and day/vocional settings, utilizing direct feedback, role-playing, modeling and behavior modificion techniques.
- Maintains current and complete client records in accordance with program standards, corpore, licensing and regulory policies.
- Collects and reports da relevant to rehabilition goals and administrive policy.
- Prepares communicion book, trement plan and other clinical documention as assigned.
- Arranges for or transports clients to all planned activities in the community.
- Assists in the planning, scheduling, implemention and evaluions of social, emotional, recreional, vocional and reled therapeutic programs for the clients.
- Develops and implements, with supervisory input, schedule to ensure adeque direct care coverage and assistance in emergency coverage.
- Assists with emergency coverage and crisis intervention for all clients as needed and participes in rotional on-call program

Minimum Knowledge and Skills required by the Job

Educion and Experience:
- High School Diploma
- Three years of rehab experience, or Associes Degree or technical certificion/licensure with one year reled experience or equivalent, or Bachelors Degree in reled field, no experience required.
