Position Overview
As an Adult Case Manager/Health Home Coordinator, you will work closely with adults living with Serious and Persistent Mental Illness (SPMI) and co-occurring disorders. You’ll help clients navigate services, build individualized goals, access resources, and strengthen both behavioral and physical health supports.
This position offers a combination of community-based work, client advocacy, and care coordination.
What You’ll Do
- Build supportive working relationships with adults experiencing mental health and substance use challenges
- Complete and coordinate comprehensive bio-psychosocial assessments
- Develop individualized Plans of Care alongside clients and treatment teams
- Coordinate care with medical providers, behavioral health professionals, and community supports
- Assist clients with accessing housing, benefits, healthcare, transportation, and other essential services
- Support crisis planning, stabilization, and transitions between levels of care
- Advocate for client rights, independence, and recovery goals
- Participate in team meetings with clients, families, and providers
- Provide transportation assistance and support community-based activities when appropriate
- Maintain accurate and timely documentation in the Electronic Health Record system
- Complete authorizations and reporting requirements in compliance with MaineCare and DHHS guidelines

Qualifications
Required
- MHRT/C or Provisional MHRT/C Certification
- Valid driver’s license and current vehicle insurance (100,000/300,000 coverage)
- Ability to travel throughout Washington County and meet clients in home and community settings
- Strong communication, organization, and problem-solving skills
- Ability to maintain professional boundaries and confidentiality
- Comfort using electronic systems and basic office technology
Preferred
- 1–2 years of experience working with individuals living with mental illness
- Familiarity with LOCUS assessments and SPMI populations
- Experience working within multidisciplinary treatment teams
