Job Overview
Seeking a dedicated Licensed Practical Nurse (LPN) to provide direct patient care in a long-term care/skilled nursing setting. The ideal candidate will be experienced in medication administration, patient monitoring, documentation, and delivering compassionate resident-centered care.
Required Qualifications
- Active LPN License
- Long-Term Care or Skilled Nursing experience preferred
- Strong clinical assessment skills
- Excellent documentation and communication abilities
Key Responsibilities
- Administer medications and treatments as ordered.
- Monitor and document resident conditions.
- Assist with development and implementation of care plans.
- Collaborate with RNs, providers, and interdisciplinary teams.
- Maintain accurate clinical documentation.
- Respond to changes in resident condition.
- Support activities of daily living and resident care needs.
- Ensure compliance with facility policies and safety standards.

Pay: $2,000.00 - $2,100.00 per week
