Job Description
Exciting opportunity to work with folks moving from homelessness to stable housing. Monday through Friday position with flexible hours. This position works within Rental Services, under the Landlord Liaison program as a Care Coordinator facilitating the move in process and connecting clients to support services, including long term case management, if necessary, to ensure success in their housing placement. This position requires collaboration with tenants and landlords with heavy emphasis on housing stability, mediating communication between tenant and landlord and problem-solving barriers that may arise. Additionally, this role with receive and process applications for Wrap Around funding to meet the urgent needs of adults with Severe and Persistent Mental Illness (SPMI) that cannot be met through the regular systems of care.
