Duties
- Conduct comprehensive assessments of residents' social, emotional, and behavioral health needs to inform personalized care strategies
- Schedule and attend weekly care plan meetings
- Utilize motivational interviewing techniques to motivate residents toward positive change and goal achievement
- Proven experience in social work and long term care, with a focus on behavioral health.
- Demonstrated success in crisis intervention and crisis management within community or clinical settings
- Prior experience in addiction counseling or substance abuse treatment programs preferred
- Strong communication skills with the ability to build trust and rapport with diverse populations
