Position Overview
The Adult Mental Health Case Manager plays a crucial role in identifying, assessing, and evaluating services for individuals, ensuring their health and safety to promote independent living. Their focus is on recovery strategies, community support, and advocating for client rights while encouraging self-advocacy.
The Role: How You’ll Make an Impact
We are seeking a kind, professional, and self-motivated Community Case Manager to join our dedicated team. In this role, you aren’t just managing a caseload; you are a lifeline for clients, helping them navigate complex systems and guiding them from crisis to sustainable, long-term independence.
Key Responsibilities
- Advocate & Support: Build trusting, respectful relationships with individuals and families, helping them identify their personal goals and unique strengths.
- Navigate Resources: Connect clients with critical agency and community resources, including housing, mental health, and social services.
- Collaborate: Partner closely with a robust network of local community providers to coordinate holistic, comprehensive care.
- Assess & Plan: Develop personalized crisis and safety plans, helping individuals and families establish stability and independence.
What We Are Looking For
- Education: A Bachelor’s degree in Social Work, Counseling, Special Education, Child Development, Psychology, or a closely related field is required.
- A MHRT-C or MHRT-C Provisional certification is required for this position.
- Skills: Strong organizational skills, a collaborative team-player mindset, and a solid working knowledge of local community resources.
- Requirements: A valid driver's license, reliable transportation, and successful completion of background and driving checks prior to hire.
