Description:
AMHC is excited to announce our new opening for a Children's Behavioral Home Health Coordinator(BHH)/Targeted Case management in our SJV area. This person will assist children and families to attain the skills and resources necessary to successfully function in the living and learning environment within the home and/or community. The Children's BHH Coordinator/TCM will establish therapeutic relationships with clients and prospective clients according to the philosophy and values of the program. This includes engaging “hard-to-reach” and “hard-to-engage” clients and their families. The chosen candidate will be expected to demonstrate the core values of AMHC by showing Integrity, Compassion, Excellence, Service, Collaboration, and Respect to co-workers, clients, and community members.
Qualifications:
- Bachelor's degree in a relevant field is required
- Valid driver's license and reliable transportation
Responsibilities & Expectations
- Assess the strength and needs of the family using Compassion and Respect, to support in linking, coordinating, and managing the families services
- Lead the treatment team in meeting the family's needs with a focus being Collaboration and Excellence
- Support the family in identifying the goals and treatment for the children with Integrity and being Service Focused
Schedule:
- This is a 40-hour per week position
