SUMMARY
The Cognitive Retraining Specialist is responsible for providing facility based individual and family therapeutic services to clients. They are also responsible for assessing the ongoing need for cognitive therapeutic services the client’s current level of care, in the residential setting.
ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to sisfactorily perform each essential function listed below:
- Maintains a caseload of clients and provides direct client services.
- Evalues the needs of the client and develops an individual rehabilition plan (IRP) using a team approach.
- Crees, coordines and evalues service plans through partnerships with stakeholders based on assessed client needs.
- Provides case management with tention to insurance needs.
- Assesses referred clients and leads development of service plan in conjunction with trement team and funding sources in accordance with corpore, contractual, and regulory standards
- Participes in team conferences and schedules and leads conferences.
- Participes in field and individual supervision with the Clinical Supervisor.
- Completes all relevant documention in an accure and timely manner as specified.
- Maintains and updes the client’s clinical records and progress note/case management record book.
- Maintains and updes client and communicion books.
- Maintains and updes on-call book and participes in the on-call process as scheduled.
- Performs other reled duties and activities as required.
SUPERVISORY RESPONSIBILITIES
Minimum Knowledge and Skills required by the Job
The requirements listed below are representive of the knowledge, skill, and/or abilities required to perform the job:
Educion and Experience:
- Bachelor’s plus a CSW certificion or a teaching certificion.
- Master’s degree in human services field preferred.
- Requires a minimum of one year experience in the field working with clients with development disabilities or mental health issues.
Certifices, Licenses, and Registrions:
- Licensure and/or certificion in field required as determined by site contractual agreement and ste standards.
- Current driver’s license, car registrion and auto insurance if providing transportion for individuals receiving services.

- Medium Work.Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
