Job Summary
The Behavioral Social worker manages a caseload of patients with psychiatric diagnoses and
their families as part of an interdisciplinary team; contributes to the overall program by
facilitating individual, group, and family meetings; leads groups; makes managed care calls; and
establishes patient/family discharge/disposition plans
Essential Duties and Responsibilities
• Supports and promotes the mission and values of Covenant Health Ministry.
• Completes Case Manager (biopsychosocial) assessment within 72 hours of admission.
• Completes admission documentation, including appropriate releases of information as needed.
• Develops individualized treatment plan and documentation for each assigned patient, with interdisciplinary treatment team, within 72 hours of admission.
• Meets with each assigned patient within 24 hours of admission, then at least every two days thereafter. Must review the treatment plan with the patient and gather their signature.
• Contacts referral source within 2 working days of admission and regularly throughout treatment
• Facilitates family meetings and multi-family programs with each assigned patient (minimum of once/week) as required.
• Facilitates group meetings as scheduled (starts and ends group on time).
• Provides immediate crisis intervention services to patients as needed.
• When requested, intervenes when the patient is dangerous to self or others, utilizing least restrictive interventions, but may include seclusion and/or restraint under the direction of an RN.
• Completes comprehensive discharge plan which includes, but is not limited to: treatment
• recommendations, medication follow-up with physician; appointment with outpatient care provider; and placement as necessary.
• Communicates with other disciplines daily regarding individual patient progress or treatment issues.
• Actively participates in quality initiatives and performance improvement to improve patient outcomes.
• Adheres to professional code of ethics.
• Complies with staff conduct and confidentiality policies.
• Maintains state and federal confidentiality regulations.
• Adheres to state guidelines of mandated reporting of child, adult, and/or elder abuse (i.e., physical, mental, emotional, and sexual abuse).
• Maintains all required competencies and completes annual compliance courses on time.
• Maintains all required licensure and certifications.

Job Requirements: (education and experience/preferred and required)
Job Knowledge and Skills
• Strong written and verbal communication skills.
• Strong interpersonal skills. Ability to handle high-stress situations.
• Crisis Prevention Institute (CPI) training preferred or obtained before completion of orientation.
Education and Experience
• Bachelor’s degree required.
• Current Maine LSW license required.
An equivalent combination of education and experience that provides proficiency in the areas of
responsibility listed above may be substituted for the above education and experience requirements.
Working Conditions/Physical Demands
Must possess the physical and mental abilities to perform the tasks normally associated with
this position that involve a combination of sitting, standing, walking, bending, stooping, and
reaching. Lifting/carrying frequently up to 20 pounds. Some stress is related to a high level of
responsibility for quality care. The ability to speak, read, write, and understand written
instructions in English is required.
