Job Summary:
Day One is seeking a dedicated Case Manager to join our dynamic team. The ideal candidate will provide patient-centered care and support to individuals within the community as well as our youth residential facilities. This position requires a strong understanding of community resources, experience in mental health support and utilization management. The Case Manager will work closely with clients and their natural supports to develop personalized treatment plans, monitor progress, and ensure connection to necessary resources and assistance.
Duties:
- Conduct intakes within given timeframe of case assignment.
- Meet regularly with caseload for face to face or zoom sessions as required (travel required within assigned area)
- Complete all necessary documentation and maintain client records in accordance with licensing and agency policy
- Collaborate with clients to create treatment plans and evaluate ongoing progress with obtaining treatment goals and objectives.
- Collaborate with multidisciplinary teams, including but not limited to, medication management providers, clinicians, referral sources, evaluators, resource management agencies.
- Coordinate with client care services, including discharge planning, home healthcare, and Primary care physicians
- Complete ongoing Kepro authorizations
- Facilitate communication among patients, families, and healthcare providers to ensure continuity of care.
- Some travel to the WIndham office required
Skills:
- Strong attention to detail, organization and time management skills
- ability to communicate clearly
- Experience and comfort working with those living with mental health and addiction
- computer proficiency
- Familiarity with HIPAA regulations and ability to maintain confidentiality.
- Excellent client care and communication skills.
- Proven ability to work effectively in a fast-paced, dynamic environment.
- ability to work well both independently and as part of an interdisciplinary team.
Education/Certification Required
Current MHRT-C or ability to obtain this certification
Bachelor's Degree in a Human services related field (required to work in Childrens Case Management)
