Case Manager

MaineHealth is seeking Case Managers to play a vital role in coordinating care across the continuum. In this role, you will advocate for patients and families, support safe and timely transitions of care, and collaborate closely with interdisciplinary teams to achieve the best possible outcomes.
Key Responsibilities
- Coordinate and manage patient care plans across inpatient and/or outpatient settings
- Assess patient needs, barriers, and available resources
- Facilitate discharge planning and transitions of care
- Collaborate with physicians, nurses, social workers, and community partners
- Educate patients and families on care plans, services, and available supports
- Ensure compliance with regulatory, quality, and utilization standards
Qualifications
- Education: Bachelor’s Degree preferred, with a specialization in Psychology, Behavioral Health, Social Work, Special Education, Counseling, Rehabilitation, Nursing, or closely related field preferred.
- License/Certifications: Current applicable state(s) license as a Licensed Social Worker preferred. Valid driver’s license and reliable vehicle required.
- Experience: With a bachelor’s degree: Two years of experience in a community health setting, including one year of case management experience preferred. In lieu of bachelor’s degree: Three years of directly relevant experience in a healthcare setting, including 6 months of case management experience. Experience collaborating with state and federal agencies, community agencies, municipal, and public programs preferred.
